| Wellness Coordinator-Dementia Unit | | | | Lucy Corr Village | Must be energetic and creative at recreational program planning. Must have experience with developing, coordinating and implementing recreational therapy programs for residents with dementia/Alzheimer. Must be responsible for participating in weekly care plan meetings and will be responsible for resident assessments, quarterly documentation and MDS reporting.

Minimum Education and Experience: Core curriculum for a bachelor’s degree, an associate’s degree, a high school diploma/GED, or a diploma in Recreation, Occupational Therapy, or related Human Services field. At least 1-2 years experience as a recreational program coordinator. At least 2-3 years of experience working with the dementia population; or an equivalent combination of training and experience.
